interior documents release late Friday show that managers at the Environmental Protection Agency were cognisant of the potential for a catastrophic “ shoo-in ” at Gold King Mine , The Associated Press reports .

Among the written document is a June 2014 employment order for a planned cleansing that noted the old mine had not been accessible since 1995 , when the entrance partially collapse .
